Advocacy is a vital component of social work practice. We advocate for the rights of individuals and communities to achieve social justice. Our social workers and peer support professionals ensure that all people have access to resources and opportunities to help them meet basic needs.

Aurora Community Health brings compassionate, knowledgeable guidance to individuals and families trying to navigate the healthcare system. Our primary purpose is to plan, develop, organize, implement, evaluate, and ensure the medically related emotional and social needs of the each individual receiving services are met/maintained.
Socioeconomic disadvantage is a fundamental cause of morbidity and mortality. One of the most important ways that governments buffer the adverse consequences of socioeconomic disadvantage is through the provision of social assistance. Our Licensed Social Workers or Certified Peer Support Specialist interact with patients, families, hospital staff, and community agencies to assist during the admission and or discharge process.
We bring compassionate, knowledgeable guidance to individuals and families trying to navigate the healthcare system. The primary goal for our social workers is to provide support for our clients who need help with paying for child care, finding food from state programs like WIC or SNAP, healthcare needs and finding help with heating, rent, utilities, mortgage and more.

The peer support specialist delivers peer support services to our clients by establishing positive rapport, sharing lived experiences or recovery stories to in still hope, foster trust and build mutuality. The peer support specialist works as a member of an integrated team at Aurora Community Health with the goal to provide support focused on advocacy, coaching and mentoring.
In collaboration with the Community Outreach Director the peer support specialist implements and support individuals with a variety of recovery, health, and wellness services in order to promote greater independence, community integration, and recovery from health issues, mental illness, and/or addiction.
Client services maybe provided in office, and or in the client’s homes. The objective is to promote a strengths-based, wellness-informed, client-centered approach to assisting individuals in achieving person-driven and self-management health goals.
