Our clinical team serves as a extension of patient's primary care team provides meaningful interactions to make the provider and the member lives easier. We focus on health disparities and gap closure by providing year round in home point of care testing completed by a nurse practitioner. We relieve some of the administrative duties and care coordination processes that can sometimes stretch the capacity of primary providers outside the clinic office, hospital or emergency department.

A RAF score is a measure of the estimated cost of an individual's care based on their disease burden and demographic information. The RAF score is then used to calculate payments to healthcare organizations.
The PMPM: the payment amount a provider receives for a patient enrolled in a medical advantage plan regardless of services provided. Healthier Patients have a lower RAF score while sicker patients have a higher RAF score which impacts the calculated payment amount, Scores are calculated on an annual basis.
